SKILLED NURSING

In-Home Skilled Medical Care: In most cases, in-home skilled medical care services have been specifically prescribed by your physician.
Skilled medical care is virtually an extension of the medical treatment and care received in a hospital or other medical facility but is provided in the comfort of the home environment.

PHYSICAL THERAPY

Physical Therapy is a rehabilitative service for the preservation, enhancement, or restoration of movement and physical function that has been threatened or impaired by disease, injury or disability. Physical therapy utilizes physical activity, exercise, physical modalities (massage, electrotherapy) assistive devices, and patient education and training to enable optimal performance and enhance health, well-being and quality of life.

OCCUPATIONAL THERAPY

Occupational Therapy is a rehabilitative service for people of all ages and diagnoses that helps them promote health and prevent injury, illness or disability with a focus on the tasks of daily living. Occupational therapy targets these daily skills with modification or adaptation of the activity, sensory processing and integration, strength, balance, coordination and visual perceptual enhancements.

SPEECH THERAPY

Speech-Language Therapy is a rehabilitative service that involves the treatment of physical and/or cognitive disorders that cause difficulty with communication and/or swallowing. Speech-language pathologists help patients across the lifespan to develop their language and speech skills and increase their ability to communicate.

SOCIAL WORKER

Social workers help individuals understand and locate available social services. They provide advice and assist people in locating resources for a higher quality of life. These social workers have direct access to a variety of information, such as housing programs, job counseling programs, job training services, financial assistance programs, and child care options. A lot of social workers also help individuals access drug and alcohol rehabilitation programs and mental health counseling. Social workers commonly act as motivators in assisting people become more stable, both financially and emotionally. They often inform clients about eligible programs and help them with the application process.

HOME HEALTH AIDE SERVICES

Our home health aides perform a variety of solutions to the daily needs of adult patients.
Services include, but are not limited to the following*: ​

• Dressing                                                                   

• Incontinence care                                                    

• Oral hygiene and skin care                                      

• Preparing meals and feeding                                   

• Assisting with mobility and transfers                        

• Obtaining pulse, temperature, and respiration         

• Performing personal care (bathing, grooming, etc.)
